Community Health Connections (CHC) is a multi-site, non-profit community health care center offering Urgent Care, Primary Family Medical and Pediatric Care, Preventative and Restorative Dental Care, Oral Surgery, Behavioral and Mental Health services for children and adults, Substance Use Disorder Treatment, specialty services including optometry eye care, optical shops, acupuncture, nutrition consultations and podiatry. CHC is mission-driven, providing compassionate, high quality health care regardless of income or health insurance status. CHC serves all – insured, underinsured and uninsured, low income, publicly housed and homeless populations. Founded on February 25, 2002, we have over 23 years of experience as the only Federally Qualified Health Center (FQHC) serving low income, underinsured, uninsured, publicly housed, and homeless populations in 35 cities and towns in North Central Massachusetts.
Quest Labs at Greater Gardner CHC is closed for the day. 1/14/26. Patients are welcome to visit any of our other health centers to complete their labs.

Experiencia negativa: despite good wonderful care from many of the practitioners here some sub genius appears to be in charge of all urgent care communications. And seeing as how I am an SME in all of those areas, see my most recent experience below! I really hope someone in a position to understand how IDIOTIC & UTTERLY UNPROFESSIONAL & NOT TO MENTION UNKIND this is and takes some action or passes along to someone else who can take some action. Or contact me, lol! Seeing as how under the new “wonderful“ phone system won’t let me reach a human without INTERMINABLE waiting time, I still have the sinus infection plus need the other work done including the blood work. The app won’t let me schedule an urgent care. I just need a callback ASAP to find out if I can only do walk-ins at this point or if I can schedule anything in Leominster. Gratefully yours, David

Experiencia negativa: My appointment was scheduled for 2:30 PM, and I arrived 10 minutes early. However, patients who came after me were taken in before me, and I had to wait over an hour past my appointment time to be seen. It’s clear that the nurses and doctors prioritize Spanish-speaking patients and their acquaintances first. This is blatant discrimination and completely unacceptable
Experiencia negativa: I am giving a ZERO STARS!!& waited 3 hours on hold then they pick up and hang up no one calls you back no on answers the dang phone this is absolutely absurd need to make an appointment for an elderly woman aka my mom is like pulling teeth and her Dr is the WORST!!!!! He shakes more then she does smh time to take a good loooong hard look at your services providers nurses and receptionist.. TIME TO DO BETTER FOR OUR COMMUNITY

Experiencia fantástica: I needed an emergency tooth extraction and they took me right in. Within 45 minutes of being there, the tooth was gone and so was my pain. The dentist was super nice and didn't make me feel bad about having so many issues with my teeth.

Had to take time out of work and order ride$ 2 times just to find out the only 1 person they have on staff wasn’t in and they don’t make appointments, so big fail like many of the CHC connections. Need better communication and staffing! Won’t be back!!

Experiencia negativa: Avoid seeing Desire Kangsen wouldn’t give referral for obvious issues had to complain to the main office who referred me to another doctor and gave me the referral immediately and apologized for her behavior. Extremely unprofessional and told me multiple times to “just go to the er” which when I did all I was told was to follow up with my PCP (which was Desire) who did absolutely nothing. If you care about your health do not see this clown

Experiencia negativa: This location frequently cancels appointments and then won't renew prescriptions because it needs a new appointment. When I ask why my appointment was cancelled, it is always the same excuse that the doctor was out of the office. How about having enough staff that you have coverage? The front desk staff is also frequently unprofessional and will talk negatively about coworkers in front of patients in line waiting to check in. Double check your appointments because they will also book you out of the wrong office.

Experiencia fantástica: My first visit today to CHC/Urgent Care on 165 Mill Street in a Leominster. Very heartfelt thanks and much gratitude to Nema at the Front Desk; Brianna the Medical Assistant; and Pinaman the PA. They were so caring and listened with empathy. I tested positive for the Flu and they were supportive and provided quality care in a clean environment. The wait period was very reasonable and I felt cared for in the best way possible. Experiencia negativa: It's getting frustrating to get hold of anyone on their phone for refills or appointments!!! Am out of my BP meds and I can't get through!!!

CHC offers MYChart, a secure patient portal, where patients can login 24/7 and request medication refills, leave a non-urgent message for your care team, view test result and make appointments, etc. The Communications Team has forwarded your review to our Medical Team. Experiencia fantástica: My husband visited the urgent care and saw provider Pinaman Gyimah-Affram. He said she was so very nice and made him feel like a human being, after being quickly dismissed by another urgent care elsewhere. She took the time to listen to him and explained what was going on with his medical condition.

Experiencia negativa: We were transferred here after MOC bought and closed our lovely pediatricians office. First visit with our daughter I knew it was a no go. The waiting room for the pediatrician was the same waiting room as the urgent care. There was a needle disposal receptacle only feet away from where we sat waiting for our appointment. Which I believe it’s great for them to exist, just not in the same space that I need to bring my child into. There are very long wait times on the phone, and at times I’ve waited for 30 minutes on hold just for it to disconnect once the phone actually starts ringing. We are transferring our daughter to a different pediatrician ASAP.

Experiencia fantástica: I visited this urgent care a little over a week ago, and I was in a bit of emotional distress because if the pain I was in. The wait time was extremely brief. The attending physician was knowledgeable, extremely informative, and compassionate. Best urgent care experience Ive had within the community.
Experiencia fantástica: When we arrive at the building's parking lot, there is a noticeable improvement in the structure and service. When we enter the clinic's doors, we are greeted by the attendants with a beautiful smile on their faces, the nurses collect basic information and everything with great affection. Primary doctors always pay attention to the smallest details, concerned with offering quality treatment. I would like to thank the team at the Clinic and the Dental office, it would be unfair if I gave a score lower than 10. Luis - Leominster

Experiencia negativa: Por dos ocaciones mi esposo y yo visitamos la oficina nueva de Leominster y supuestamente no está en el sistema después de 8 años bajo el cuidado médico con ellos y las Front Desk no están cualificadas para atender pacientes deberían ir a training se ríen del paciente hacen comentarios en relación al paciente. Quisiera saber a donde fue la información personal de mi esposo no protegieron la misma.

Experiencia negativa: The only thing nice about the Leominster office is a few providers (Michelle Boudreau and Jessica Gray and VERY few nurses/MA) I know some employees who are looking for new jobs cause of other employees and that’s just sad!!!! The call center is an absolute joke and when it gets transferred to the site it’s even more of a joke they r very quick to refer you to the ED for a UTI and tell you that ur allergic to a med when it’s one of the only ones you have ever been prescribed and what’s listed as an allergy is a completely different med!!!! Even the people at the front desk aren’t pleasant to deal with!!! It’s sad cause this is health care and at times I don’t even wanna call cause of how they treat people Cmon CHC you can do better maybe training classes on how to treat patients who are sick and not 100%!!! You work for a drs office and should treat patients with compassion and not attitudes!!!! If this continues I might look into finding a new pcp!!!

Experiencia negativa: Never finished my root canal in early march. Half way through they said “sorry we’re running out of time so we’re going to have to schedule another appointment to finish it” they then scheduled my appointment for 3 months later. Two weeks into june. Then they canceled and offered me an earlier date that I couldn’t stress enough that I wasn’t going to be able to make it. So they canceled and didn’t reschedule it until end of December. So I never showed up because of their unprofessionalism. I ended up seeing a root canal specialist today and now they have to do a ton of work because they never completely cleaned out my molar and now I have to spend a ton of money because of them. I would avoid this place if you can. I should’ve. I regret not going anywhere else.
Experiencia positiva: Unhappy that you get to your appointment early and they make you wait over 1 hour only to be seen for 15mins! Also when you ask for a referral they make you wait weeks and do not send over all the correct information needed! The doctors are nice but I'm not sure if I want to continue with the office. (It's the same for the Fitchburg office) they overbooked the appointments!
Experiencia fantástica: I recently went to the Dentist here and I have to say Dr. Patwardham and his assistant Julie were great. I absolutely hate going to the dentist but they made me feel so comfortable and relaxed. I Have quite a few cavities and needed a few fillings and both Dr. Patwardham and Julie took the time to walk me through their game plan. I have a few more appointments coming up and I'm not nervous anymore. I will continue to go Dr. Patwardham for everyone of my dental needs. Thank you guys so much for all the help
Experiencia fantástica: I had a decent experience with Dr Patwardhan in the dental area. He was very adamant about saving my tooth, but I knew I had to lose it because I couldn't wait weeks for a root canal. I appreciated him trying everything, however. He was good about my anxiety too. And very informative. The dental assistant was great too! She asked my pronouns and corrected the dentist for me. I really appreciated them both.
Experiencia negativa: AVOID AT ALL COSTS. This place is an absolute joke. They never pick up the phone and when they do their receptionists give you some of the biggest attitude for asking simple questions. Their scheduling is a mess, several times we have shown up for appointments for them to be cancelled with no notice, they never tell us when doctors are out. They will literally make you mental health issues worse.
Experiencia fantástica: Today, I had an emergency of a split tooth and went to The Community Health Connections, in Leominster, MA. Dental Office. When I arrived at my appointment with my broken molar, I was so nervous; I wanted to cry. Knowing the state of my tooth, I was hopeless. My tooth had a huge filling and was badly broken. Dr. Patwardhan and Julie took control of the situation right away assuring me of a positive outcome. They showed empathy and confidence while explaining the solution. When they started to work on my tooth my anxiety melted away. I was impressed by the outstanding manual dexterity of Dr. Patwardhan, but also by their passion for providing care for those in need. Dr. Patwardhan is fast, precise, detail-oriented, and in my honest opinion, is a true master of the art of dentistry. The level of skill is exceptional. When I noticed the perfect state of my tooth, I was in awe. I was truly impressed with my visit, the staff is very friendly, professional, and attentive. The office is modern, clean, and the care is first class. They took care of my emergency without hesitation. I recommend this office because of the quality of care and the skill level of Dr. Patwardhan, Julie, and the rest of the staff. They are a great team and work extremely efficiently together. I had a great experience and this office is now my go-to dental office.

Experiencia fantástica: I highly recommend the Leominster CHC. The receptionist are great & Very friendly. The dental assistance are on point & Dr Patwardham is a excellent dentist! One of the best dentist I’ve ever had! Great personality, comfortable environment & always answers any questions. Best of all Dr Patwardham is gentle with the needles to anyone who may not like needles. You will be in great hands at the Leominster CHC, especially with Dr Patwardham!

It really showed the potential for high quality knowledge that made me very satisfied. guided me in the best treatment and I was very happy with the patience and wisdom in his words. the receptionist Carmen as always friendly and a great service with that usual smile! she even called me asking if I would like to start the treatment, right after I left the office because a client didn't show up. I really liked her calling me worrying about the toothache I was feeling and trying to fit me in as soon as possible! An excellent job!! I recommend this office!

Experiencia fantástica: Five Stars for customer service, hygienists, and dentists!! Dr. Pat is professional, friendly and gentle. The work I had done there is great. His assistant Julie is fantastic. They accept all insurances including MassHealth and have an in-house sliding fee; there is no excuse to put off dental work because of finances - they work with everyone. I would recommend this office to anyone!
Experiencia negativa: Worst doctor ever doctor Medipally Mahavi she never remember what happens last appointment she give me antibiotics and 2 days later I call because I feel so bad and says what happened, said feel bad because the antibiotics and she ask me who give me the antibiotics ♀️ You!!!!!! And she said why I give you antibiotics? OMG then others doctors like Elizabeth do similar things no one read your medical history no body care about the patients we should report them !

Experiencia negativa: Doctors are okay but sometimes you have to wait for 30 mins just to check in as they only have one person in check in and one in check out but lines of patients. Also getting ahold of my doctor is impossible. I call the line and wait on hold for 30 mins just to have them tell me that they’ll inform a nurse to call me back....they really need a direct line to your PCP. However the 3rd floor Lab tech is awesome. She’s funny, courteous and caring. She was a lot more helpful with speeding up my testing than my actual doctor.
Experiencia negativa: Leominster Community Health Center is one of those offices that take on too many patients and the quality of care suffers for it. Get them in and out as quickly as possible seems to be their motto. I have not had any issues with dental, but I refuse to go back to their behavioral health and their doctor's offices.
Experiencia negativa: the doctor Medipally Mahavi, was not very attentive to my health issues, i got a feeling she waned to get rid of me the sooner the better. On top of that she gave me wrong diagnose.
Experiencia negativa: Worst doctors office I have ever been to. Not only was the doctor (Doctor Medipally) extremely rude, she has scarred my son for life. First of all we were waiting for a LONG time to be seen... a good hour before the doctor even came in the room. My son was five. He’s been raised right and sat there and was very well behaved, like he usually is. Well her response to that was “so your son has been very well behaved, so he’s medicated right? What’s he on.” Ummmm, nothing he’s just well behaved and are you serious about that statement. Kids nowadays can’t be well behaved unless they have ADHD and they’re on aderall and Ritalin huh? Then we got into talking about school and I told her that he had previous education and he was in the Head Start program for preschool. Her response “people don’t usually send their kids to preschool unless there is something wrong with them. Umm what? Last I checked people sent their kids to preschool to prepare them for kindergarten and the rest of school. So after insult after insult she pulled the last straw and called my son Overweight in a very demanding way. A year later he is calling himself fat. She told him he needed to lose five pounds. He was FIVE... Five pounds over weight really? You really had to one bring that up when all little kids have a little chub and really make a big deal about him losing five pounds? The way she went about that was awful. So now a year later I need to listen to my son call himself fat and say that he needs to lose weight. Then came the shots... they FORGOT WE WERE IN THE ROOM AND HAD US WAITING FOR TWO FREAKING HOURS for this nurse. I switched offices with another CHC office in Fitchburg because of this visit. I will never go back to that place. Worst doctor and worst visit EVER! So thanks for scaring my kid with body issues for the rest of forever... I appreciate it, NOT... DON’T GO TO THIS PLACE IF YOU WANT GOOD CARE AND TO BE TREATED WITH RESPECT AND DIGNITY! They don’t seem to know what that is or how to run their office...

Experiencia negativa: At first I liked this place with the doctors being nice.. Until the last few weeks.. Two weeks ago I went in and had 3 molars taken out on a Tuesday by Saturday night I was in the er with a fever of 101 er doc said it became a dental abcus. That Monday I called back for an appointment to get seen and was told to come back in at 8 am on Tuesday which was a week later I get up there talked to the person at the desk that I wasn't on the list to be seen and now that I have gotten most of the infection out my self and the gums aren't as swollen there's a nerve sticking out.. I'm so not happy about this

Only down fall is making appointments are a pain in the butt and the wait time to be seen is bad. Example: if appointment is for 10am you have to wait till least 20 mins AFTEr to be seen
