Fantastic experience: I really enjoy going to these offices. It does not feel like a hospital or cold doctors office. Everyone there is helpful. I see Michael Sullivan there and he has been one of the most helpful doctors I’ve ever seen. Also, the phlebotomist are the best around.

Fantastic experience: This has been a heaven-sent facility for me at this stage in my life. Dr. Spurs took the time to listen to my concerns and the issues I was experiencing, she immediately ordered testing and I returned for a visit for her to explain my results. Dr. Spurs addressed my deficiencies and I also met with the dietitian, Leslie. The staff is friendly and I would recommend Forum for overall health and wellness.

Fantastic experience: So very greatful to have found Susan Harris and her team. When other practicioners try to treat symptoms, Susan's team researches the root cause. There are no other functional medical offices near my home in Keller. (And I have looked!) I drive 20 miles to Coppell....it's well worth the drive for good healthcare!

Fantastic experience: Forum Health Coppell is a wonderful clinic. The staff is very friendly and professional. The providers are very professional and very knowledgeable, especially when it comes to female and male hormones. They offer many services, treatments and also have a variety of a top of the line supplements. I have been a patient of Michael Sullivan for over 16 years. He has helped me through pre, peri and post menopause and I am very satisfied. He takes a holistic approach to help his patients along with bio-identical hormones.

Negative experience: I went to Forum health because my A1C was 5.9 and BS was 99. The nurse practitioner put me on berberine . The berberine reduced my A1c to 5.7 but my blood sugar went to about 123. I was 72, 115 lbs with osteoporosis. I was in process of doing all to reverse my osteoporosis with exercise , food, supplements, vibration machine, resistance and weights etc. I realized that berberine reduces muscle AND bone mass. Hello. It interfered with my next bone scan. So i ditched the berberine, ditched Forum health, used Ceylon cinnamon and my A1C is 5.7 and BS 93. 2 bone scans later my bone loss numbers are finally improving no thanks to Forum Health

Fantastic experience: Lori Spores is amazing! I had my thyroid removed 9 years ago at age 32 when cancer was found. I have struggled ever since then and saw 4 endocrinologists before coming to see Lori. They all would do nothing except mess with my dosage of Levothyroxine and add and take away Liothyronine. No one ever changed the medication or looked at other factors. When I started there, Lori immediately started me on a different medication (what a concept!) And has been working with me to figure out what my body is doing. Since I'm a complex case, it's a process, but my numbers are finally moving in the right direction. After 9 years!

Fantastic experience: Thank you to Lori Spors, who I had been seeing for Hashimoto and hypothyroidism. If I hadn't been going to a provider who ran full panel thyroid blood tests, I would not have found out my thyroid cancer diagnosis. When Lori noticed my elevated blood test and thus an inflammation, she sent me to get a thyroid ultrasound ASAP, where 3 nodules were found in the gland. I was then referred to a thyroid specialist to get a biopsy on them and that's when I found out about having thyroid cancer. It was caught early, thankfully. Thank you for being so thorough.

Fantastic experience: In April of 2024, I found Lifestream Health Center which is now called Forum Health Coppell. I was 52 and overall was just feeling like something was off. I had all the textbook symptoms of someone my age and needed help. A friend recommended that I go get my hormones checked and to get a full bloodwork panel. I found Lifestream as I was researching online and was anxious to call and get an appointment, but it was the BEST decision ever! That was the beginning of this 15 month journey to feeling great! At my first appointment I finally felt like I was being listened to and felt validated in the symptoms I was experiencing. I wasn’t going crazy! My bloodwork showed many deficiencies in levels and we went through each area and looked at where I should be. A plan was set to start hormone pellets, to undergo a complete detox using GDRx to rid my body of infection and inflammation, and to trim down and focus on my overall health & nutrition with supplements. It was an all hands on deck approach and each and every provider I met with has helped me to get my spark back. I am thankful for my results. My energy is back and I am sleeping great at night. Fantastic experience: I have been a patient of Michael Sullivan since 2013 and he is the BEST! His nurse Kiesha is the BEST nurse ever! They are both so very kind, very understanding, super super helpful. They both go above and beyond and they truly do care! I can't say enough about how good they are. Yes, sometimes it's a bit of a wait but they're worth it and they always give you their full attention. They are not distracted by their phones like other people are. I live more than an hour away but I refuse to see anyone else! I highly recommend Michael and Kiesha! :) My husband and I have followed him wherever he goes. They're worth it! They are very good people with very good and caring hearts. Believe me, Michael and Kiesha do not disappoint.

Fantastic experience: I'm someone who's suffered with a lifetime of unexplained chronic health issues, and I've seen many doctors, both traditional and functional medicine. I can say that this is one of the best offices I've ever worked with. They take you seriously and go above and beyond to try and help you, which already sets them apart. But even more than that, this is the best experience I've had in terms of systematically running tests to identify the root cause with confidence. I've seen other doctors who would say things like "oh, you probably have gut issues" and give you treatment without doing any actual testing to verify that. With Forum Health, I did several rounds of testing that produced negatives at first, which was a little surprising. But instead of stopping there, they continued on to test other possibilities and were able to identify something. I've spent years speculating on what I may or may not have, but now I am armed with the confidence of knowing for sure that I don't have particular things, or that other things may truly be causing my symptoms. A lot of chronic conditions are not always understood well enough yet to be able to identify, and many don't have very effective treatments. So I know that what they do is very difficult, and even despite best attempts it's not always possible to identify or cure everything. But having worked with many practitioners, I can say this practice has gone far above and beyond the norm and I'm extremely hopeful to be working with them!

Negative experience: Train your front desk people better! These people do not pay attention to important details like when to schedule a cortisol test or what bloodwork you’re actually coming in for. I showed up for a bloodwork appointment at 9:15 AM to be told that they couldn’t do my cortisol test because it needed to be done between 7 and 9 AM. No one told me that when they scheduled my appointment or when they checked me in. The annoying thing is I could’ve easily come in earlier had they let me know, but they screwed up when they made my appointment. Nobody at the front desk offered an apology either. One girl just said to another that she should’ve noticed when she checked me in, but she did not apologize to me. I was just rescheduled, and that was it. What kind of business or practice messes up majorly and then says nothing at all about the inconvenience?! When I went back to do this blood test, the same front desk lady I dealt with last time misquoted what the cash pay price would be because she assumed I was getting a full blood work panel done, even though she had the piece of paper with my lab orders on it right in front of her. She tried to tell me the total price would be in the $200s but really it was $98. So she unnecessarily stressed me out ironically right before I took the cortisol test. Also, they overcharge for bloodwork in general. You’re better off going directly to Quest. After multiple bad experiences, I will not be back.
Negative experience: This office can’t do anything right to save their lives. I have been trying to get my birth control prescribed to me for months and they have sent the wrong prescription twice and every time they say they will send another prescription to the pharmacy, nothing happens. Crazy that a gynecologist office can’t prescribe a simple pack of birth control correctly. Don’t go here if you need any prescriptions. They are AWFUL!

Negative experience: Broken system. They'll call 90 mins before the appointment to get paid and if they can't get a hold of you, they'll cancel your appointment without telling you. EDIT after owner response: I received an automated msg "confirming my appointment", yes. And that was all. 90 mins before the appointment, they wanted to get paid. I was in a meeting and missed the call. Their voicemail said to call a number. I called them back 20 minutes later. No Answer. I left a message. No call back. No messages to say anything was wrong. I waited for my teledoc call for an hour and a half before trying to call them again. NO ANSWER. I then got automated txt message asking if it could help. And only then did they tell me I was cancelled. But if I wanted to pay, they would reschedule me. The guy on the phone did not care. He insisted they did everything right and contacted me several times before (but those were automated messages and nothing to follow up on) except for the ONE SINGLE call just before my appointment. No cancellation notification. And zero empathy that they screwed up my schedule for the day. He just says, "so you want the appointment or not?" Your "multiple attempts" do not include automated messages confirming my appointment. You made ONE personal phone call and left ONE voicemail. I followed YOUR instructions to call you back and you never tried again.

As a rule we always make multiple attempts to confirm appointments with patients before canceling, we apologize if there was any confusion or inconvenience in this instance.
Negative experience: My insurance covers labs, I alerted the staff of this but they insisted that they collect cash and will bill insurance. They never billed. Then, after my labs were done they refused to give me results unless I went back in to pay more out of pocket to see them. They also catch you off guard and charge you again at the end of the visit and claim it goes towards your deductible, never happened. I saw them in match, Seems fraudulent. Don’t waste your time at this place, it’s a joke and they take your money and run. I’m not the only review on here who had this issue. This clinic needs to be SHUT DOWN.

It seems there may have been some misunderstanding about insurance and we sincerely apologize if we didn’t explain things in a way that was easy to understand. Negative experience: I was referred to this office and have to say I was thoroughly disappointed. I came in for my first visit to address some health issues and it was brief and labs were recommended. An in-person appointment was scheduled to go over lab work. Unfortunately, I never got to see the provider due to the front office staff scheduling my appointment wrong. I literally called them the day before the appointment and they confirmed my appointment time, only to later text me that the provider never works that late and I would need to reschedule. But this appointment was set, confirmed and I took off work for it! They blamed it on the "new hire" but that's a weak cop-out for bad training by management. Why would I trust you with my body, if I can't trust you with an appointment? Luckily, I found a new provider, I'm very happy with, and had my labs transferred.
Upon reviewing your case, it appears there may have been a misunderstanding regarding the nature of your appointment. Our records show that it was scheduled as a virtual appointment, and unfortunately, we did not receive a call to cancel or notify us otherwise. However, we regret if this was not communicated clearly to you. As a practice, we strive to provide the highest level of care and service, and your feedback is invaluable in helping us identify areas for improvement. We are actively working with our team, including new hires, to ensure our processes and communication are consistent and reliable moving forward. Negative experience: This facility used to be good. It seems that after being bought out by Forum things have gone south. One cannot get appointments with a consistent provider, therefore, "Swiss cheese" healthcare. No one keeps up with when you need bloodwork or a mammogram or a pap smear. Also, they will not give you a detailed bill to give to your. insurance company for reimbursement. They say they cannot provide due to the fact they are paperless. This makes no sense as if paperless then email me my bill in full detail with your: Name of provider 1 tax id number 2 description of service 3 procedure code billing 4 charge for services provided 5 diagnostic codes My issue with them has been dragging on since 2/24.

Fantastic experience: This office is SO CARING and makes a way to help you with whatever you need. If you need a vitamin IV with glutathione and you've got covid, you can pull your car up in the driveway and they will wheel the IV out to you. I love that Susan is up on the latest tests and treatments from an integrative standpoint. She is supportive of nonsurgical alternatives to common medical issues and offers a thorough perspective. The office is beautifully decorated and makes an ordinary doctor's appointment feel like a treat. There is really not another place like this.

Negative experience: I highly do not recommend. They overcharge, then don’t return your calls or portal messages. They don’t send orders, or follow up as they say they will. The provider doesn’t review your medication list or have it on hand prior to, or during the visit. I will not be referring future patients here.

Negative experience: The replies from corporate are lies. None of my issues have been resolved or addressed when brought to their attention. They way overcharged me and have not remedied the situation or mailed me a refund. They have claimed to do so twice and both times I never received anything. Never go here!
Negative experience: This is one of the worst customer service experiences I have ever had with a medical office. I was overcharged when my insurance provided me coverage and they never alerted me or told me and kept the funds as an account credit. It took over 5 times calling to get someone to tell me what was going on and say they would send me a check for a refund. I never received the refund and it has been over 2 months. I have tried calling and sending messages through the portal with no response. Most recently, I saw they scheduled a follow up appointment for me which I did not make. Unfortunately the lack of communication and radio silence has led me to making this review. I would not recommend this Clinic to anyone that would like timely responses or clear communication.

Fantastic experience: I highly recommend Michael Sullivan. I’ve seen him since my first diagnosis of hashimotos in 2019 and wouldn’t go to anyone else. The testing they do is whole person based and important. He’s optimized my hormones and given advice when I’ve been at the end of my rope with all my symptoms because I have so many. It’s a tough journey, don’t go it alone! The office here is great, the best part is plenty of free parking and great coffee next door at George. Great little community with a playground close by. I had to bring my 3 year old with me due to a babysitter illness and all the staff was wonderful and able to accommodate her. I appreciate not being over prescribed medications, and the bills are much lower than some similar places in Dallas. Most places fully just don’t take insurance. Thank you Forum!

Fantastic experience: I have felt very tired, achey, dizzy, and depressed for two years and did not not why until I came here. The extensive lab tests they did finally found out what was happening to my body. They took the wholistic approach to treating me instead of just writing me scripts that would possibly treat the symptoms but not the cause. I paid for the labs because it was worth it to me to possibly feel better. If you want a quick cheap doc to write a script then it’s not the right place for you.

Fantastic experience: This was my first visit to lifestream the house is cute and cheery. I was greeted by the front desk with a smile, even though I was late, the check in process was pretty simple. The nurse was quick and efficient. Jana Roger’s was very very knowledgeable And made me feel very comfortable. The phlebotomist was outstanding. I’m an extremely hard stick and she got it on the first try. Nutritionist was extremely helpful and non-judgmental. The office manager even stepped out and asked how everything was Overall, my experience, outstanding highly recommend

Negative experience: Wow, where do I start? This was the worst most embarrassing most humiliating appointment of my whole life I have never ever felt so small and sunk in …THEY DID A BAIT AN SWITCH ON ME …I had several people around me at the front desk, talking loud about what I owe that I didn’t even ask for it was so degrading all these people in this office and not one of them could get my visit, correct… I called on the phone prior to my visit for testing kit. I was given a quote for the testing kit when I got there, You take my insurance card you sent me in a room with a doctor that I didn’t even ask for I was only there to get the kit I even let the young lady who did my triage know also. The doctor figure out what test i’m talking about she left out and another lady came in with a whole Nother price…I had the $350 for the Test they are the ones who added another hundred dollars onto the test and also $150 for an office visit that I didn’t ask for…I was told that I could pay some and pay the balance when I come back BY THE DOCTOR Roger’s for the results in which I didn’t need to because I had $350 on me .. WELL! all that changed in a matter of the instant they were trying to was charged me for an office visit that I didn’t ask for about $150 , so now my $350 kit went from $350-$600 and some change.. but it doesn’t make any sense why would I pay $150 just for somebody to order a kit off-line and ship it to me and I do my own testing at home?? that is crazy!!! The Doctor just talked to me then all of a sudden I owe $450 plus $150 for a talk with a doctor that I didn’t ask for when I kept saying that I am only here to pick up the test. I was told it was $350 and I can come and get it. wow, this was so humiliating.. then you have the office manager who comes and gets me and sits me in another room trying to explain a paper to me like I’m some child and I don’t understand how things go I get it I came based on what your staff said and when I got there, you guys change the rules I didn’t.. so the young lady at the front desk was very adamant about me paying $147for the office visit and I told her no just like I told the office manager….then she said well you have $350 you were going to pay for that test so you have $147 that you can pay for your office visit I said no let me correct you I bought $350 to pay for the test and that’s what the $350 is for when I leave from here. I will still have my $350 in my pocket going to get my test kit Don’t try to Bully me . This was so sad. All of them were so disconnected and not on the same page. Nobody Knew anything at all Well, I won’t be going back there at all and I have spent my $350 somewhere else and got the test I was looking for… Wow such shame such embarrassment not only for me but them too

Negative experience: My wife went and had a good experience but I did not, I had to ask so many questions and ask for certain things as I new my wife’s experience as a reference. Felt like they rushed my time with me and did not listen to everything I had to say along with did not take the time to explain their findings and results. Ended up being extremely expensive with staff being short and impatient. The nutritionist was helpful but everything was an added cost and when I went to get the supplements they recommended it would’ve been $200… I went to Tom Thumb and got the same size and organic quality for a total of $62! Was very disappointed with the quality of service and price. The texting feature seemed nice but never lead to actual help as they said they would just get back to me. Susan was polite but too quick and is not a Dr so is limited. Thanks

Positive experience: Overall love LIFESTREAM and have been happy with the care I receive from Susan. I strongly dislike the lab they use- Medlab 24 - because they are out of network with most insurance and it’s not cheap. The lab also starts texting debt collection threats BEFORE they even send a bill. When I go to an in-network provider, I don’t want to worry about large bills from a lab that doesn’t work with my very common insurance (UHC) and can’t manage its billing practices. My question is, why would most insurance companies not accept this lab in their network??

Negative experience: So disappointed in Lifestream. My husband and I both went to get a functional medicine approach on thyroid and hormone health (for me) and overall wellness check for my husband. We had to pay another copay for the follow-up appt with Dr Harris (times 2 for each of us even though we were on the call together). All of which we were fine with bc I thought we were getting exceptional care/diagnosis/ holistic, functional medicine approach. Instead, she recommended a Detox and passed him on to their nutritionalist. What a joke he is. He never sent the email he said he was going to send with recipes and shopping list to prep for the Detox. It’s been 2 weeks since my husband finished his program and not ONE call or email to see how it was going and still nothing. No call or follow up from Susan Harris either. We could’ve bought the program online but bought from Lifestream bc thought it would come with support. Dr Harris also said she was going to give the name of who she refers for small cyst removal. After i realized i never got it, I emailed my patient portal. No reply. A week or so later, I called the office for the info. The receptionist said she didn’t know why that office hadn’t reached out yet and would contact them. Never heard back from either. Also, be prepared to pay for bloodwork out of pocket (unless you have BCBS) since they use an independent company. Lifestream gets a lot of praise on FB and I know there have been happy patients. I did my research and was excited to see her, but i wont go back. Do your research and ask them about all the upfront costs with your insurance before you go. The follow up has been horrible. Makes us both feel they just wanted to sell their Detox program. She said she wanted to get to the root of the problems for my husband but neither she nor nutritionist has reached out for post detox assessment or bloodwork. What a waste of extra costs and copays, which I gladly paid thinking we were getting a functional medicine approach.

Negative experience: I am a physician. Read this story to the end please: If I could give them zero star still it is a lot. Be aware of overcharging there. I had a well visit yearly gyn routine check up ( pap smear and blood work) and checked their facility to be in network with Aetna and up to now I paid 310$!!!!!! It is fraud. Aetna just claims that routine check up is free but the day of visit they charged me $163 and claimed it goes through your deductible!!!! Big lie. It never showed up in my insurance claims and yesterday just texted me to claim 150$ more after 6 months for a well visit gyn visit. Stay away of them. I did the pap smear and some of their lab works again as the results were not reliable. Nobody tells you what items would be checked in your blood work just they provide it as a package and boom, wait for a flood of bills every months.

Susan was the one person who helped me, after 7 months of me looking for someone to fix my issue. She is very professional, nice and cares about her patients. The day I went she saw the issue and got me what I needed. I left there overfilled with joy. Highly recommend them.

Fantastic experience: I have been coming to LifeStream since January of 2018. In my very first appointment with Susan Harris, NP, she was able to discover things about my medical and total physical conditions that doctors who had seen me for years could never figure out. I have chosen to accept the excellent advice to get better by digging deeper into the root causes of each of my medical conditions. This advice included receiving IV Therapy two times a week for 12 weeks and maintaining a healthy diet and lifestyle. As a result of following Susan’s guidance, my labs are better and I feel and look healthier. The entire team at LifeStream is phenomenal! They go out of their way to ensure that every stone is unturned and each possibility is researched to understand the full picture of the health of their patients. From Susan and her excellent team of nurses to the front office team to the lab techs, each one makes it a point to get to know the patient and to assist them. Very few health and medical offices put the patient first these days. LifeStream does. Every patient. Every day.
Fantastic experience: I cannot say enough good things about Susan Harris and her staff. Susan is compassionate and truly loves what’s she does and that carries over to her patients. I have seen other physicians but continue to come back to Susan as she looks at the underlying issues and does not just treat the symptoms. Fantastic experience: Wish I could rate a million stars! Every staff member is so kind and considerate and makes you feel at ease from the moment you walk in until you leave! The environment is fun, inviting and makes you feel like you're at a close friend's house instead of a doctor's office. I got a Hydrafacial from Chala about 2 months ago and my skin is still crystal clear. I HIGHLY recommend trying a facial from the med spa. Best place ever!
Fantastic experience: The service and staff here at LifeStream is fantastic. I've had a therapeutic IV when I was sick, and also had a Hydrafacial done by the esthetician Chala Frieze. Both times I went into this wellness center I was greeted so sweetly and felt at home. This place deserves more than 5 stars.
Fantastic experience: I have been very happy with my whole experience at LifeStream. The office is very pretty and the location is quite charming with beautiful landscaping. The staff has been wonderful to work with, everyone from the girls that greet you to the nurses and the nurse practitioner, Susan Harris. I really like the extensive lab work and follow up they did. I also like that they recommend bio-identical hormone replacement and as much natural medicine as possible. I would definitely recommend this office and staff to others.

Fantastic experience: I love Susan and the entire team at LifeStream! They care about the patients and treat you like an individual. I feel like they are thinking about me and my health even when I'm not there. They dig into things to offer solutions to root causes, not just treating symptoms...although they will do that in conjunction with searching for the root cause. They really look at the whole patient and do everything they can to help you live a well and healthy life!
Fantastic experience: I've had a great experience with Susan Harris and her entire staff. Since I first started, I've had great success as I've lost 42 lbs with their weight loss program and coaching.
Negative experience: BEWARE!!! I went into them complaining of fatigue... they said, let's do some blood work and see what is going on. That seems fair.... They use a company called "True Health Diagnostics"... They billed my insurance company for $13,533.66 for blood test. I called them and said this must be a mistake. "No that's correct, don't worry, you won't have to pay that amount, they work it out with the insurance company." Well, I'm so offended. I feel like they ordered test that were unnecessary. This company "True Health Diagnostics" has been under investigation in the past for paybacks to doctors to order more test than needed. I believe everyone should be paid, but I don't believe in trying to rip-off insurance companies, just because you can. My health is fine, I was feeling fatigued, and now I am sick. SICK that they are billing my insurance company over $13,000 - for a bunch of blood test that were not needed.

Fantastic experience: I highly recommend Cryo sessions at Lifestream. The ladies that work here are amazing, always friendly and helpful. I was scared to try Cryo and put it off for a long time, then I finally tried it when I injured my hip (bursitis). Someone stays with you in the room during the 2-3 minute session to talk you through it and it makes the time pass really quickly. I tried it because I just wanted to get the inflammation in my hip down to lessen the pain. After only 3 or 4 sessions I felt so much better that I was able to start running again and just completed a marathon I thought I would have to pull out of. The same injury had me sidelined for 8 weeks last summer! It is also very refreshing during these crazy hot summer days.
